You do not need to print a paper copy of this form. It can be
downloaded (In Firefox, right-click on the link and click Save Link
As... In Internet Explorer, right-click on the link and click
Save Target As...), filled out, saved and e-mailed directly to George Oryschkewych at goryshkewych@cox.net. Please remember to drop off your payment at the Y by the due date
and time or your swimmer(s) will not be entered in the meet.
Adobe Acrobat Reader version 7.0 or higher is required to save
the file. If you have an older version, you will need to
uninstall the older verion before installing the newer version.
When saving the document (click File ->Save As...),
include the swimmer's last name and meet name in the title of the
document. For example, "Goodwin-Sprint Spectacular.pdf"

information packet prior to the entry deadline. The meet will be
removed from this page once the entry due date has passed.
